Famous failures of complex engineering systems

There were a lot of famous failures of engineering systems: Titanic, Tacoma narrows bridge collapse, Subsynchronous resonance in power systems, Telephone and power system outages, Estonia Ferry sinking, Denver airport baggage handling system, and Ariane 5.
